AI-Driven Advancements: Optimising Chronic Disease Care Through Personal Health Record Management

    Abstract

    Chronic diseases present a formidable global health challenge, necessitating efficient management through accurate and accessible health records. The problem statement identifies the obstacle of managing health records for chronic patients remains complex due to data fragmentation, limited accessibility, and a lack of patient-centeredness. Addressing these challenges requires innovative solutions to optimize PHR management and enhance chronic disease care outcomes. The research reveals the challenges faced by chronic patients such as interoperability issues between different healthcare IT systems, patient engagement, privacy, and security concerns. The proposed solution is an application that serves to analyze different categories of personal health records and to provide prior preventive healthcare or preventive recommendations based on each patient’s health profile, followed by self-health check-ups for assistive healthcare based on symptom tracking feature which can be delicately shared with patient’s doctor with utmost privacy. This app aims to securely manage, store, and access their personal health information, empowering patients to take an active role in their healthcare management. It also opens different channels for leveraging new technologies, including artificial intelligence, to promote efficient personal health record management.
