Description
This project brought together a multidisciplinary team to co-design and co-create a holistic digital platform for integrated dementia self-management support. The platform integrated personalized goal planning for persons living with dementia with broader dementia support and services from the Department of Health, as well as those provided by charities and dementia support groups. This approach facilitated communication, engagement, and shared decision-making among persons living with dementia and their formal and informal carers.
The team included the core academic members and key stakeholders such as persons living with dementia, their informal carers, relevant clinicians like dementia nurses, occupational therapists, general practitioners, and decision-makers from community healthcare organizations and the Department of Health.
